I have attempted to use the plugin by changing the relevant directories with the default terms suggested by you. It works but disables the MaxCDN via W3 Total Cache. If I use only one main CDN url that you allow, there is no way to optimize the site. Plus, the site becomes slower. I usually use multiple CDN urls to speed up the site.
I discovered that the main problem is the change of directory names, which Total Cache doesn’t recognize. What can I do to make Total cache recognize the new folder names (wp content, includes, etc)? Could you test using custom files in W3 as shown in the attached screenshot and let me know what works?
If you can’t fix it, you might as well refund my money as the plugin would be useless to me. Else, I will write a comprehensive bad review because it’s disappoint.

domain.com/SKIN/ ==> cdn.domain1.com/SKIN/
domain.com/STORAGE/ ==> cdn.domain2.com/STORAGE/
domain.com/LIB/ ==> cdn.domain3.com/LIB/
domain.com/MODULES/ ==> cdn.domain4.com/MODULES/SKIN, STORAGE, LIB, MODULES are the new paths for wp-content/themes, wp-content/uploads, wp-includes, wp-content/plugins respectively. These should be changed base on your current HMWP configuration.
Total cache should automatically recognize new paths. If still the issue then let us know the path/url that isn’t being recognized.

Please let us know your site’s wp-admin login details as private reply so that we can have a look at the issue and fix it for you.

Can you check please? And the replacements for cdn are to be added in “replace in html” option under replace tolls tab (not htaccess).
